Comments:

An unannounced onsite monitoring inspection was conducted on 2/15/2022 from 1:15 p.m. to 4:55 p.m. and 2/16/2022 from 9:10 a.m. to 11:15 p.m. A COVID pre-screening was conducted prior to the inspection. The Lead Caseworker participated in the entrance conference, inspection of office setting, and remained available during the inspection. The Lead Caseworker reported a census of 21 foster children and nine (9) approved provider homes. The inspector reviewed three (3) foster child and one (1) provider home records. Caseloads and office setting were inspected. An exit interview was conducted with the Lead Caseworker and two (2) Caseworkers on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on record review, the agency failed to address required elements in the Social History for Foster Child 2 (FC2). Findings: 1) The Social History for FC2, dated 11/9/2021, did not include information about the education and occupation for the child?s parent, grandparents, aunts, and uncles. 2) The findings were reviewed during the exit interview. 3) Staff 3 reviewed the record and was interviewed. 4) Staff 1 and 3 (S1 and S3) acknowledged the findings.
