Comments:
An unannounced monitoring inspection was conducted on November 15, 2019 from 10:35 a.m. to 4:16 p.m. and on November 18, 2019 from 9:13 a.m. to 10:38 a.m. The agency reported eleven provider homes and seven foster children in care on November 15, 2019. Two provider homes and one foster child record were reviewed. It was reported that no new staff have been hired since the last inspection. Staff S1 was available to respond to questions during the inspection. Preliminary inspection findings were reviewed during the exit meeting conducted at the conclusion of the inspection with Staff S1. An acknowledgement form was signed.

Upon the receipt of the violation notice, the licensee should identify the necessary corrective action and develop a plan of correction for each violation. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; position(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Please do not post the Supplemental form. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Standard #: 22VAC40-131-180-D
Description: Violation:
Based on a review of the record for foster home FH1 and interview with Staff S1, the licensee failed to ensure the required interviews were conducted.

Findings:
1. FH1 was approved by the licensee on 05/01/19. The home study completed by the licensee for FH1 documents 2 of 3 required face-to-face interviews.
2. During an interview with the Licensing Specialist, Staff S1 acknowledged that the third interview was not documented in the record as being completed.

Plan of Correction: Site Director will retrain Foster Parent Coordinator on licensing standards and ensure audits of all Home Studies will include a minimum of the 3 required home visits prior to certification.

Standard #: 22VAC40-131-180-J-1
Description: Violation:
Based on review of the home study completed for foster home FH2 and interview with Staff S1, the agency failed to include all department-required information.

Findings:
1. FH 2 was approved on 05/31/19 and review of the home study failed to include verification of the current marital status. This regulation requires verification of marital history and status to include divorces.
2. During an interview with the Licensing Specialist, Staff S1 confirmed that verification of the current marital status not included in the home study.

Plan of Correction: Site Director will retrain Foster Parent Coordinator on licensing standards and ensure audits of all homes prior to certification have the required marital history (including divorces) and the forms in the files.

Description: Violation:
Based on review of the record for foster home FH1 and interview with Staff S1, the licensee failed to ensure a home study addendum was completed.

Findings:

1. Review of the completed home study indicated FH1 did not have a pet.
2. Review of the home environment checklist for FH1 revealed a vaccination document for a pet (canine).
3. During an interview with the Licensing Specialist, S1 confirmed that a home study addendum should have been completed to include the addition of the pet and to address the safety of the pet around children.

Plan of Correction: Site Director will retrain Foster Parent Coordinator and Case Manager on the licensing standards to add a Home Study addendum in the event the family adopts a pet while certified, and review all opened homes to add ensure all pets are included in an addendum to the home study.

Standard #: 22VAC40-131-190-S
Description: Violation:
Based on a review of the record for foster home FH1 and interview with Staff S1, the agency did not obtain documentation or verify that the provider had received a pet license, as required by law.

Findings:
1. The home study narrative and home study documentation for FH1 did not contain verification that the canine in the home had received a license, as required by law.
2. During an interview with the Licensing Specialist, Staff S1 acknowledged that this information was not in the home study for FH1.

Plan of Correction: Site Director will retrain Foster Parent Coordinator and Case Manager on the licensing standards and ensure all animals (even ones adopted post certification) have documentation in the file the pet?s license from the locality in which they live.
