Type of inspection: Monitoring. Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/04/2022 10:00 a.m. to 2:49 p.m. and 10/05/2022 at the Fredericksburg office for personnel records review from 10:03 a.m. to 2:45 p.m. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of children in care: 1 Number of approved provider homes: 4.The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 1.Number of foster home records reviewed: 1. Number of staff records reviewed: 1. Number of interviews conducted with staff: 2. An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit meeting. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Description: Violation:
Based on review of the record for foster home FH1 and interview with agency representative AR1, the licensee failed to include all required elements in the home study.

Findings:
1. FH1 was approved by the licensee on 04/18/2022.
2. The record for FH1 failed to document verification of divorce for both foster parents.

Plan of Correction: Director/QA Specialist will retrain Foster Parent Coordinators

Description: Violation:
22VAC40-131-180.J.2.f Based on review of the home study for foster home FH1 and interview with agency representative AR1, the licensee failed to ensure that the home complies with standards for the home as required by 22VAC40-131-190.

Findings:
1. 22VAC40-131-190.P requires that approved providers shall arrange for responsible adults to be available to serve in the caretaker?s role in case of emergencies.
2. The approved provider in FH1 indicated she would serve as caretaker in case of emergencies.
3. AR1 confirmed there was no documentation of a responsible adult or adults to be available to serve in the caretaker?s role in case of emergencies.

Description: Violation:
Based on a review of the record for foster child FC1 and interview with agency representative AR1, the licensee failed to include a required element in the child?s individualized service plan.

Findings:
1. The plan did not include the target date for the child?s discharge from the program.
2. AR1 confirmed that this required element was missing

Plan of Correction: All staff will be retrained on the requirement for each child to have a target discharge date in the ISP
