An unannounced monitoring inspection was conducted on December 17, 2019 10:44 a.m. to 3:54 p.m. The agency continues to provide independent living arrangements (ILA). At the time of the inspection the agency reports 4 youth placed in ILA. During this inspection, two personnel records and two youth records were reviewed. An exit meeting was conducted with the Executive Director and Case Manager to discuss preliminary inspection findings before leaving the agency on December 17, 2019. An acknowledgment form was signed. Please make a copy for your files once you sign the documents and please return the signed and dated documents to this office via scan and email (preferred method).
