The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Additional Comments/Discussion:
After an inspection of the office setting and conditions, an entrance conference was held at the agency on 6/14/23 with the Executive Director. The Executive Director and the Senior Case Manager were available and accessible during the inspection. A youth?s record and staff records were reviewed. On 6/15/23, a preliminary findings meeting was held with the Executive Director and the Senior Case Manager in order to seek clarification about a few documents.

An exit meeting was conducted on 7/7/23 to review the inspection findings.

The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.

The department's inspection findings are subject to public disclosure.
