Comments:

Type of inspection: Renewal A renewal) inspection was initiated on 5/21/2024 and concluded on 5/22/2024. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was (6). The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (#1) resident. record, (2) staff records, submitted by the facility to ensure documentation was complete. Provider has not had any updates to policies and procedures and had two new hires since last inspection visit. An exit interview was conducted with the CEO on 5/24/2024, where findings were reviewed, technical assistance was provided and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Description:	Based on review of Foster Youth record FY#1, the medical examination reports submitted for review to the licensing inspector did not provide evidence that Foster Youth #1 was free from communicable diseases to include tuberculosis . 1. Foster Youth #1, date of placement with agency was 1/3/24. 2. Medical examination reports submitted for review from Entity #1 documented two separate visits for Foster Youth #1 occurring on 11/22/2023 and 2/6/2024. 3. Medical examination reports dated 11/22/23 and 2/6/2023, submitted for review to licensing inspector did not determine or document that Foster Youth #1 was free from communicable diseases, including tuberculosis on or prior to 1/3/2024 placement or up to 60 days following emergency place child. 4. The findings were reviewed during the exit interview.
