Comments:

A monitoring inspection was initiated on 9/27/2021 and concluded on 9/29/2021. The interim program manager was contacted by telephone to initiate the inspection. The interim program manager reported that the current census was 28 children in care and 22 approved provider homes. The inspector emailed the interim program manager a list of items required to complete the remote documentation review. The inspector reviewed records for 3 children, 3 provider homes, and background checks for 4 employees.al provider home. The interim program manager and program director were interviewed during the inspection. An exit interview was conducted by telephone with the interim program manager and program director on 9/29/2021 at approximately 1:50 pm where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
