Comments:
An unannounced monitoring inspection was conducted on 06/13/2022 from 10:30 am to 3:40 pm and on 06/14/2022 from 9:10 am to 2:15 pm.The Acknowledgement of Inspection form was signed and left at the agency.

The agency reports a total of 26 children in care and 20 approved provider homes. During this inspection, 3 children records, 3 provider home records and 1 staff record was reviewed. Background checks for 2 additional provider homes were reviewed. Three employees were interviewed during this inspection.

An exit meeting was conducted to review the inspection findings.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-190-A-3
Description: Violation:
Based on review of the record for Provider Home (PH) PH5, and interview with the program manager, the licensee failed to ensure the home provided sufficient appropriate space and furnishing for each child receiving care in the home including: bed furnishing equipped with clean, safe and comfortable sleeping furnishings.

Findings:
PH5 was approved by the licensee on 02/25/22 for the placement of 2 children. The initial certificate of approval documents approval for the placement of 2 children.

The record of children placed in PH5, documents a total of 5 children were placed in the home from 3/31/22 to 4/3/22.

PH5's record documented an addendum dated 3/30/22 increasing the capacity to 5 children. The addendum documents that the 2 older children will sleep on twin mattresses in the spare room and the 5th child will sleep on a "fold out" also in the spare room.

During an interview with the program manager it was acknowledged that 5 children were placed in PH5 that included children sleeping on mattresses which are not bed furnishings.

Plan of Correction: Program Manager will staff in detail with the case worker and the foster family file specialist prior to approving any short-term foster care, to assess the foster family?s ability to provide sufficient and appropriate space and furnishings for each child that would receive care in the short term foster care home.
