Comments:
An unannounced monitoring inspection was completed by the Licensing Specialist on 1/29/20 from approximately 10:59 a.m. to 4:26 p.m. This location has a capacity of zero (0) as this is the corporate office for Intercept Health/YouthQuest. This office maintains the personnel records for all licensed YouthQuest offices across the state. This location is separately licensed as it does perform child placing activities, specifically, intake screening for all offices.

The following is a listing of the activities for this inspection:
Two staff records were reviewed. These two staff records consist of the staff members whose background checks were scheduled to be updated since the July 2019 monitoring inspection.

The Human Resources Compliance and Training Analyst was available and accessible during the inspection. An exit meeting was conducted with the Human Resources Compliance and Training Analyst and the Executive Director at approximately 3:41 p.m. An Acknowledgement of Inspection form was signed and left at the corporate office.

Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.

Violations:
Standard #: 22VAC40-191-40-D-1-c
Description: Violation: Based on interviews with staff and review of a personnel record, the facility failed to comply with the requirements for background checks before three years since the date of the last criminal history record check report for staff, S1.
Findings:
1) The Licensing Specialist reviewed S1?s personnel file and discovered the following-
a. The received dates on the criminal history record check reports confirms the current report was not obtained
before three years since the date of the previous report.

2) The Licensing Specialist interviewed S2 about the criminal history record check.
a. S2 was not able to provide a background check conducted before three years since the date of the last
criminal history record check.
b. S2 agreed with the Licensing Specialist?s findings.

Plan of Correction: Executive Director will ensure Human Resources has adjusted the
tracking measures used to track 3 year check due dates and have
a running spreadsheet tracking sent and completed dates for each
of the required checks (name search, sworn statement, CPS, and
Fingerprints). Tracking calculations will now be tracked 3 years
from the 1 st item sent for processing, with results expected to be
received within 30 days prior to the due date to remain in
compliance with the Regulation.
