Comments:

This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 6/2/21 and concluded on 6/14/21. The Executive Director was contacted by email on 6/2/21 to initiate the inspection. This location has a capacity of zero (0) as this is the corporate office for Intercept Health/YouthQuest. This office maintains the personnel records for all licensed YouthQuest offices across the state. This location is separately licensed as it does perform child placing activities, specifically, intake screening for all offices. The Licensing Specialist emailed the Executive Director a list of items required to complete the inspection. The following is a listing of the activities for this inspection: A staff list was reviewed by the Licensing Specialist to determine if there was a need to review personnel records for new staff. After doing so, it was determined no new staff members were hired since the December 2020 inspection. The Executive Director was interviewed on 6/7/21. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.
