Comments:

A monitoring inspection was initiated on 11/22/21 and concluded on 12/09/2021. The Executive Director was contacted by telephone to initiate the inspection. This location has a capacity of zero (0) as this is the corporate office for Intercept Health/YouthQuest. This office maintains the personnel records for all licensed YouthQuest offices across the state. This location is separately licensed as it does perform child placing activities, specifically, intake screening for all offices. The Licensing Specialist emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The following is a listing of the activities for this inspection: Two staff records were reviewed. The Manager of Compliance and Development and the Assistant Director were interviewed together on 11/30/21. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.
