Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 8/16/23 from 11:04 AM ? 4:14 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: Not Applicable The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of youth records reviewed: 0 Number of interviews conducted with staff: 1 Additional Comments/Discussion: An entrance conference was held at the agency on 8/16/23. This location has a capacity of zero (0) as this is the corporate office for Intercept Health/YouthQuest. This office maintains the personnel records for all licensed YouthQuest offices across the state. This location is separately licensed as it does perform child placing activities, specifically, intake screening for all offices. Five personnel records were reviewed. The HR Business Partner was available and accessible during the inspection. A preliminary findings meeting was also conducted on the above-mentioned date with the HR Business Partner. An exit meeting was conducted on 8/21/23 to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
