Comments:
A monitoring inspection was initiated on 5/17/2024 and concluded on 5/21/2024. The Director was contacted by email to initiate the inspection. The Director reported that the current census was zero as this location is an administrative office. The inspector emailed the Director a list of items required to complete the inspection. Two staff records were reviewed, as provided submitted by the facility. An exit meeting was conducted with Director on 5/21/2024 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

The licensee can submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.


The department's inspection findings are subject to public disclosure.

Description: Violation:

Based on interview and a review of personnel record documentation, the agency failed to ensure Staff 1 (S1) met qualification requirements to serve as a child-placing supervisor.

Evidence:
1) S1 was hired 9/29/2023.
2) The record for S1 documented the staff member held a master?s degree in a field other than social work.
3) The record for S1 documented 11 months experience in an out of state child-placing agency.
4) The licensing consultant contacted Staff 4 (S4) and requested clarification regarding the calculation of S1?s experience.
5) The agency reported they determined the applicant was qualified according to 22VAC40-131-140.E.4.b, which requires four years? experience providing casework services to children and families in a child-placing agency.
6) The findings were reviewed with Staff 3 (S3) during the exit interview and S3 was interviewed.
7) The inspector and S3 discussed whether S1 would be qualified based on 22VAC40-131-140.E.4.c
8) S3 provided information about additional work experience for S1. At the time of hire, S1 did not have 5 years of experience as required by 22VAC40-131-140.E.4.c
9) As of the date of inspection, S1 had reached the required five years of experience through her employment at this agency.
10) S3 acknowledged the findings
