Comments:

An unannounced renewal inspection was conducted on November 19, 2019 from 11:21 a.m. to 3:40 p.m. and on November 20, 2019 from 10:05 a.m. to 2:27 p.m. The agency reports 6 youth in independent living arrangements (ILA), 2 foster care children, and 2 approved foster homes. During this inspection, portions of seven personnel records were reviewed, one foster home record, one foster child record, and the records of two youth in ILA were reviewed. Three staff were available to respond to questions during the inspection. Office setting and conditions were in compliance and the license was posted. Preliminary inspection findings were reviewed at the conclusion of the inspection with Agency Representative (AR) 1 and Staff (S) S1. An acknowledgement form was signed.
