Type of inspection: Monitoring
A focused inspection was conducted on 5/17/2022 from 10:00 am until 2:00 pm. The Acknowledgement of Inspection form was signed and left at the facility on 5/17/2022.

Number of children in care: 4
Number of approved provider homes: 12
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
Number of children?s records reviewed: 2
Number of staff records reviewed: 1
Number of interviews conducted with staff: 1 entrance interview and 1 exit interview with the executive director
Observations by licensing inspector: License was posted
Additional Comments/Discussion: 4 foster home records were reviewed

An exit meeting was conducted on the day of the inspection.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Description: Violation: Based on a review of a foster child file (FC-2) and an interview with an agency representative (AR-1), the agency failed to include elements in the child?s intake assessment as required.

Evidence:
1) The intake and preplacement form for FC-2 dated 10/12/21 does not include current information on the child?s health or the date of planned achievement for the permanency planning goal.

2) In an exit interview on 5/17/22, AR-1 acknowledged that the elements were not included in the intake assessment for FC-2 as required.

Description: Violation: Based on a review of two foster child files (FC-1 and FC-2) and an interview with an agency representative (AR-1), the agency failed to include elements in the children?s social history reports as required.

Evidence:
1) The social history for FC-1 dated 1/20/22 does not include the child?s developmental or medical history.

2) The social history for FC-2 dated 11/10/21 does not include the child?s developmental or medical history.

3) In an exit interview on 5/17/22, AR-1 acknowledged that the elements were not included in the social histories for FC-1 and FC-2 as required.

Description: Violation: Based on a review of a foster child?s record (FC-1) and an interview with an agency representative (AR-1), the agency failed to have a written foster home agreement for each child in care.

Evidence:
1) The "Intake and Preplacement Form" contained in the record of FC-1 documents that the child was placed on 12/21/21. There was no foster home agreement contained in the record of FC-1.

2) In an exit interview on 5/17/22, AR-1 acknowledged that there was no foster home agreement contained in the record. (repeat)

Description: Violation: Based on a review of two foster child files (FC-1 and FC-2), four foster home files (FH-1, FH-2, FH-3, and FH-4) and an interview with an agency representative (AR-1), the agency failed to keep each case file up to date and in a uniform manner.

Evidence:
1) Documents for the children?s files and the provider files listed above were maintained on a shared drive. The documents could not be accessed in a read only manner by this licensing specialist. Documents were printed for review during the inspection.

2) In an exit interview on 5/17/22, AR-1 acknowledged that they were still working on putting together hard copy files for the foster children and the foster families in their program.

Description: Violation: Based on a review of a foster home record (FH-1) and interview with an agency representative (AR-1), the agency failed to obtain a search of the child abuse and neglect registry maintained by any other state in which the individual has resided in the last five years for an adult child (AC-1) living in the home.

Evidence:
1) AC-1 completed a sworn statement dated 9/26/21 documenting that AC-1 had resided in a state other than Virginia within the last five years.

2) There was no a search of the child abuse and neglect registry maintained by the other state contained in the foster home record of FH-1.

3) In an exit interview on 5/17/22, AR-1 acknowledged that they have not received the central registry search results for AC-1. (repeat)
