Comments:
A monitoring inspection was initiated on 9/27/2021and concluded on 9/28/2021. The Treatment Foster Care Director was contacted by telephone to initiate the inspection. The Client Operations Manager provided information required to conduct the inspection. The inspector emailed the Treatment Foster Care Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed documents from two (2) foster child, three (3) staff, and four (4) provider home records. An exit interview was conducted with the Treatment Foster Care Director, Regional Supervisor, Client Operations Manager, Sr. Manager of Foster Care Administration, Admissions Manager, Foster Family Trainer Supervisor, Documentation Specialist, two Treatment Foster Care Supervisors, Family Services Specialist Supervisor, and Family Services Senior Manager on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

The information gathered during the inspection determined violations with applicable standards or law.

Violations:
Standard #: 22VAC40-131-290-K-1
Description: Violation
Based on record review for Foster Home 2 (FH2), the agency failed to obtain tuberculosis screening results in compliance with regulation for Household Member 1 (HH1).

Findings:
1. The record for FH2 contained a tuberculosis screening form indicating HH1 was screened on 4/12/2021.
2. The medical provider did not include a statement that HH1 is free from tuberculosis in a communicable form.
3. The findings were discussed during the exit interview and staff acknowledged the findings.
