Comments:
A monitoring inspection was initiated on 06/04/2024 and concluded on 06/07/2024. The Director was contacted by email to initiate the inspection. The Director reported that the current census was 15. A list of documents required to complete the inspection was provided to the Director. Two child, two provider home, and three personnel records were reviewed as provided by the agency. An exit interview was conducted on 6/7/2024 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Inspection findings will be posted to the Department?s public website within 5 days of receipt of the inspection documents. The department's inspection findings are subject to public disclosure.

The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

Description: Violation:
Based on a review provider home record documentation and interview, the agency failed to obtain required background checks before three years since the most recent background checks for the provider in Provider Home 2 (PH2).

Evidence:
1) The current central registry search result for the provider in PH2 was dated 6/29/2023. The most recent central registry search result for the provider was dated 6/25/2023.
2) Staff 11 (S11) was interviewed during the exit interview.
3) Staff participating in the exit interview reviewed the record for additional information.
4) Staff 3, 5, and 11 (S3, S5, and S11) acknowledged the finding.

Description: Violation:
Based on a review of provider home record documentation and interview, the agency failed to obtain a required background check within 30 days of Household Member 1 (HH1) in Provider Home 2(PH2) turning 14 years of age.

Evidence:
1) HH1 turned 14 on 11/14/2023
2) The central registry search results for HH1 were dated 1/31/2024.
3) The findings were discussed during the exit interview and Staff 11 and 13 (S11 and S13) were interviewed.
4) S13 acknowledged the findings.
