Comments:

Type of inspection: Monitoring A monitoring inspection was initiated on 6/11/2024 and concluded with an exit meeting on 6/14/2024. The Executive Director was contacted by telephone to initiate the monitoring inspection. The Executive Director reported that the current census was (3). The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (#2) resident. records, one was a discharge record, (2) staff records, submitted by the facility to ensure documentation was complete. Provider has not had any updates to policies and procedures since last inspection. An exit interview was conducted with the CEO and Program Director on 6/14/2024 via TEAMS, where findings were reviewed, technical assistance was provided and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations documented on the violation notice issued to the facility. Description:	Based on review of Staff record S#1 and record for S#2, sworn statements submitted for review to the licensing inspector did not provide evidence that the sworn statements were completed prior to Staff #1 and Staff #2 first day of employment. 1. Staff #1, date of employment with agency was 1/6/2022. Sworn statement in the record for employee S#1 was completed and dated on 1/6/2022. 2. Staff #2, date of employment with agency was 1/8/2023. Sworn statement in the record for employee S#2 was completed and dated on 1/8/23. 3. The findings were reviewed during the exit interview .
Plan of Correction:	Sworn statements will be completed and signed prior to the date of hire. Executive Director and Human resources personnel will ensure signatures are obtained during background application completion.
