Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on July 22, 2020 and concluded on July 27, 2020. The Therapeutic Foster Care Program Director was contacted by telephone to initiate the inspection. The agency reported 11 children in care, and 14 approved provider homes. The agency reports no new staff since the last inspection. Board Officer Records were previously reviewed and determined to be in compliance at this time. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 complete child records, 1 additional child record (discharge standards), 3 complete provider home records and 2 staff record submitted by the agency to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-200-G-4
Description: Based on the review of the provider home records for foster home (FH)-1, FH-2, and FH-3 and based on an interview with agency representatives (AR), the licensee failed to include the title of the individuals approving the home on the certificate of approvals for FH-1, FH-2, and FH-3.

Findings:
1.The review of the records for FH-1, FH-2, and FH-3 did not document the title of the individuals approving the home. The certificates had a list of titles without identifying the specific title of the persons signing and approving the home.
2.During the exit interview, AR-1 and AR-7 acknowledged that there was a list of two titles on the certificates and that the specific title of the person signing/approving the home was not identified.

Standard #: 22VAC40-131-290-F
Description: Based on the review of the foster child (FC) record for FC-1 and based on an interview with agency representatives (AR), the licensee failed to document arrangements for FC-1?s recommended follow up medical care related to a medical condition/medical condition (MC)-A.

Findings:
1.The record for FC-1 contained medical documentation (visit summary), dated 06/08/2020 which recommended that FC-1 undergo a surgery for medical condition A (MC-A). At the time of the inspection, there was no additional documentation located in the record that documented that this foster child had received this treatment or was scheduled to receive this or any alternative treatment for MC-A.
2.During the exit interview, AR-8 and AR-7 reviewed the record and attempted to locate documentation of case notes or other documentation related to the recommended medical care treatment and was not able to locate any documentation related to the receipt of the recommended treatment or an alternative treatment.
