63.2 (1) General Provisions

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on May 14, 2021 and concluded on May 20, 2021. The Therapeutic Foster Care Program Director was contacted by email to initiate the inspection. The inspector emailed the Program Director a list of items required to complete the inspection. The agency reported 17 children in care and 17 approved provider homes. The inspector reviewed 1 complete child records 2 complete provider home record, and 1 staff record. There was a previous review of Board Officer Records.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-40-M
Description: Based on review of the record for FC1 and an interview with Agency Representatives (AR), the licensee failed to provide interventions and follow up services necessary for FC1.
Findings:
1. The record for FC1 documents a contact note completed on 05/12/2021 documenting that FC1 was forced to miss days from school due to being required to go to work with his foster parent.
2. During the exit interview, AR7 reported that FC1?s foster parent works for a transportation company and was required to travel with her for work thereby missing school to the degree that the school requested a parent-teacher conference to address absences.

Plan of Correction: In response to the 22VAC40-131-(2)-40-M violation, TFC Director will train TFC staff regarding the requirement of providing interventions and follow up services when necessary and documenting appropriately.

Standard #: 22VAC40-131-290-F
Description: Based on the review of the record for FC1 and an interview with Agency Representatives (AR), the licensee failed to make arrangements for FC1 to receive recommended medical follow up care and treatment.
Findings:
1.The physical exam located in the record, dated 01/21/2021 recommended follow up care regarding medical testing, Medical A. However, as of the date of the inspection, 05/20/2021, there was no evidence of follow up care regarding this testing.
2. During the exit interview, AR2, AR5 and AR7 reviewed the record and acknowledged that there was no evidence of follow up care regarding testing related to Medical A.

Plan of Correction: In response to the 22VAC40-131-(6)-290-F violation, TFC Director will train TFC staff regarding the requirement of making arrangements for clients to receive recommended medical follow up care and treatment as well as documenting the follow up accordingly.
