63.2 General Provisions.

Technical Assistance:
Technical assistance is offered to the licensee regarding standard 22VAC40-131-370.F. During the inspection, there were several discrepancies in the dates and signature dates on several documents.

Comments:
A monitoring inspection was initiated on 10/25/2021 and concluded on 10/28/2021. The Program Director was contacted by telephone and email to initiate the Inspection. The Program Director reported that the current census was thirteen (13) children/youth in care and eighteen (18) approved providers. The Program Director reported one (1) newly hired staff since the last inspection. The Inspector emailed the Program Director a list of items required to conduct the remote documentation review portion of the inspection. The Inspector reviewed two (2) children/youth records (one of which was a discharged record), three (3) complete provider home records, one (1) additional provider home record (background checks only) and two (2) employee records. An exit interview was conducted with agency representatives on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-131-290-F
Description: Based on the review of the records for Foster Child (FC)-1 and based on an interview with agency representatives, the licensee failed to arrange for the child to receive recommended follow up dental care and treatment.
Findings:
1.The record contained documentation of a dental ?consultation form? dated 06/11/2019 which recommended that FC-1 receive recommended treatment for Medical Condition 1 (MC-1).
2.The record contained documentation of a dental ?consultation form? dated 07/15/2021 which recommended that FC-1 receive recommended treatment for MC-1.
3.At the time of the inspection, there was no evidence located in the record of FC-1 receiving the recommended treatment for MC-1 and no evidence of the licensee making arrangements for the youth to receive the recommended treatment.
4.During the exit interview, there was no evidence presented by agency representatives which supports that the licensee made arrangements for FC-1 to receive the recommended treatment for MC-1.
5.At the time of the inspection, there was no documentation located in the record addressing why FC-1 did not receive the recommended treatment.
