Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4/26/2024 an office settings inspection was completed by Specialist Sherry Woodward. On 4/22/2024 and 4/23/2024, Licensing Specialist requested remote access to review records. The exit meeting was held on 4/30/2024. The Licensing Inspector met with the Director to initiate the inspection and concluded an exit with the team listed in the summary section of this document. Number of approved provider homes: (11) Number of children in care: (13) Number of Foster Home records reviewed (2) Number of children?s records reviewed: (1) and (1) discharge record. Number of staff records reviewed (4) Observations by licensing inspector reviewing the office settings revealed the posting of the license and inspection documents was observed. There were no major changes made to the policies or procedures since the last inspection. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Description:	Violation: Based on the record review for FC-2, the agency failed to demonstrate that a follow-up visit to the primary physician was completed after a medical hospitalization per recommendation. Findings: 1. Per the quarterly progress report narrative documentation dated 12/20/2023 addressed the need for follow-up medical care after an ER visit. 2. The licensing specialist requested the medical examination follow-up documentation. 3. The findings were discussed during the preliminary findings and exit review
