Comments:

A monitoring inspection was initiated on 06/04/2024 and concluded on 06/06/2024. The Director was contacted by email to initiate the inspection. The Director reported that the current census was nine. A list of documents required to complete the inspection was provided to the Director. Two child, two provider home, and three personnel records were reviewed as provided by the agency. An exit interview was conducted on 6/5/2024 and 6/6/2024 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued-pending determination The department's inspection findings are subject to public disclosure.
