Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on June 16, 2020 and concluded on June 18, 2020. The Therapeutic Foster Care Program Director was contacted by telephone to initiate the inspection. The agency reported 16 children in care, and 17 approved provider homes. The agency reports no new staff since the last inspection. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 complete children records, 2 complete provider home records and 1 staff record submitted by the agency to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency.

Violations:
Standard #: 22VAC40-131-140-F-2
Description: Based on the review of the personnel record presented for Staff (S) 1 and based on an interview with agency representatives (AR), at the time of hire as a caseworker, S-1 did not have one year of experience in providing casework services to children and families.

Findings:
1.Based on the review of the personnel record for S-1, a ?Welcome Letter,? located in the record, dated 05/11/2018 and the ?Your Offer Letter Employment Information? dated 05/15/2018, S-1 was promoted from a ?Case Aide? position to the position title ?Family Consultant,? equivalent to CaseWorker.
2.Based on the resume attached to the ?Your Offer Letter Employment Information? dated 05/15/2018, S-1 did not have once year of experience in providing case work services to children and families.
3.Based on an Employment Application dated 02/21/2018, S-1 did not have any experience providing casework services to children and families.
4.During the exit interview, AR1, AR2 and AR3 acknowledged that at the time of hire as a caseworker, S-1 did not have one year of experience in providing casework services to children and families.

Plan of Correction: In response to the 22VAC40-131-(3)-140-F-2 violation, TFC Director will provide training to all TFC supervisors, regional supervisors, and HR regarding the requirement of having one year of experience in providing casework services to children and families in order to be a caseworker.

Standard #: 22VAC40-131-290-F
Description: Based on the review of the record for foster child (FC) 1 and based on an interview with agency representatives (AR), the licensee failed to document arrangements for the child to receive recommended medical care and treatment.

Findings:
1.The record for FC-1 documented a physical exam dated 10/31/2019 which recommended two medical follow up care items. Medical follow up item #1 was recommended to be completed within one week of 10/31/2019. Medical follow up item #2 was recommended to be completed within three months of 10/31/2019.
2.At the time of the inspection, there was no documentation of a follow up appointment scheduled or completed to address medical follow up care items 1 or 2.
3.During the exit interview, AR1, AR2, AR3 and AR6 carefully reviewed the record and were not able to locate documentation of arrangements for FC1 to receive the recommended medical care (Item 1 or Item 2). They were not able to locate evidence that FC1 received the follow up care and were not able to locate documentation of appointments scheduled or cancelled for the medical care recommended items (Item 1 or Item 2).

Plan of Correction: In response to the 22VAC40-131-(6)-290-F violation, TFC Director will provide training to all TFC staff regarding the requirement of documenting arrangements for children to receive recommended medical care and treatment.
