Comments:
This inspection was conducted by licensing staff using an alternative remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/07/2021 and concluded on 05/12/2021. The Program Director was contacted by email to initiate the inspection. There were 14 children in care, and 17 approved provider homes. The agency reports no new staff since the last inspection. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed 2 children?s records, 2 provider home records, and 1 additional provider home record (background checks only) submitted by the agency to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. Any violations can be found on the violation notice. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-40-M
Description: Based on review of the record for FC1 and interview with AR2 and AR4, the licensee failed to provide interventions and follow up services necessary.
Findings:
1.The record for FC1 documents a contact note completed on 04/20/2021 documenting suicidal ideations. The record contained an unsigned ?Safety Behavior Plan? completed on 04/20/2021 documenting the safety risk of the foster child who has a significant history of suicidal ideations. At the time of review, the record for FC1 did not document follow up interventions taken by the licensee to address FC1 contract for safety.
2.The ?Safety Behavior Plan? documented that a staff would meet with FC1 to review the plan. However, there was no documentation of the review of the ?Safety Behavior Plan? with FC1 or the assigned foster parents.
3. During an interview, the Licensing Specialist asked AR2 and AR4 if the licensee reviewed the ?Safety Behavior Plan? with FC1 and signed by FC1 as well as the assigned foster parents. AR2 and AR4 reviewed the records and were not able to locate documentation of the review of the ?Safety Behavior Plan? with FC1 or the assigned foster parents.
4. During the interview, AR2 and AR4 acknowledged that neither FC1 nor the assigned foster parents sign the ?Safety Behavior Plan?.

Description: Based on the review of the record for FC1 and based on an interview with agency representatives (AR), the licensee failed to document an interview with the child prior to the child?s placement.
Findings:
1.There was no documentation of a pre-placement interview in the record for FC1.
2.During the exit interview, AR2 acknowledged the absence of the documentation of a pre-placement interview in the record for FC1.
