Comments:

An unannounced mandated monitoring inspection was conducted on January 12, 2022 from 11:00 am to 4:30pm and January 13, 2022 from 10:15 am to 4:00 pm at the Stafford location. The following is a list of activities for this inspection: 1.Two (2) foster children records reviewed 2.Three (3) complete provider home record reviewed 3.Four (4) additional provider home records (background checks only) reviewed 4.Two (2) staff records reviewed 5.Policy and Procedures reviewed 6.A physical plant inspection was completed Staff were available for the preliminary findings/exit interview which commenced at approximately 03:30 p. m. on January 13, 2022 at the Stafford location. An acknowledgement form was signed and left with the licensee at this time. There was one (1) citation for violations of Standards for Licensed Child-Placing Agencies. Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non- compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on the review of the record for Foster Child (FC)-1 and based on an interview with agency representatives (AR), the licensee failed to document arrangements for the child to receive recommended follow up medical care. Findings: 1. Based on the review of the record for FC-1, the document titled ?72-hour Medical,? dated 09/10/2021 listed medical condition A and recommended a follow up evaluation. 2. Based on the review of the record for FC-1, there was no documentation of arrangements for the child to receive the recommended follow up medical care. 3. During an exit interview with agency representatives, AR-4 reviewed the record and confirmed that there was no documentation of arrangements for the child to receive the recommended follow up medical care for medical condition A.
