Comments:
An unannounced renewal inspection was conducted on 06/06/2022 from 10:15 am to 5:00 pm. The Acknowledgement of Inspection form was signed and left at the agency.

The agency reports a total of 20 children in care and 23 approved provider homes. During this inspection, 2 children records, 2 provider home records and 8 staff records were reviewed. Background checks for 2 additional provider homes were reviewed. Three employees were interviewed during this inspection.

An exit meeting was conducted to review the inspection findings on 06/08/2022 with the TFC Director.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and Violation Notice. The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-330-G-3
Description: Violation:
Based on review of the contact notes for Foster Child (FC) FC2, the licensee failed to ensure contacts included training and guidance provided to the foster parents.

Findings:
FC2 was placed with the licensee on 04/01/22 for treatment foster care services.
The contact notes in the record for FC2 did not document training and guidance provided to the foster parents.

Plan of Correction: In response to the VAC40-131-330-G-3 violation, TFC Director will train TFC staff regarding the requirement of ensuring contacts includes training and guidance to foster parents as well as including the appropriate documentation in the file.

Description: Violation:
Based on review of the criminal record checks, and email, the licensee failed to ensure the criminal record check was kept as required by the background check regulations.

Findings:
Background checks for Staff (S) S8 reviewed. The most recent criminal record check provided for S8 on 6/6/22 is dated 10/25/18.
An email was provided on 6/7/22 stating the repeat criminal record check in compliance with the background check regulations could not be located.
A repeat check for S8 dated 6/7/22 was provided.

Plan of Correction: In response to the 22VAC40-191-70-A violation, TFC Director will work with the HR department on ensuring the criminal record check is kept as required by the background check regulations.
