Type of inspection: Renewal An inspection was conducted on 6/06/2023 from 10:30 am until 3:30 pm. Additional records were reviewed electronically on 6/07/2023. Number of children in care: 27 Number of approved provider homes: 22 Number of children?s records reviewed: 3 Number of staff records reviewed: 4 Number of interviews conducted with staff: 1 entrance interview with the TFC Regional Supervisor. 1 exit interview with the TFC Regional Supervisor, the Senior Family Consultant, the Family Services Supervisor, the Senior Manager of Family Services, the Senior Manager of Client Operations, the Documentation Specialist, the TFC Admissions Manager, and the Senior Manager of Foster Care Administration Additional Comments/Discussion: 3 provider home records were reviewed. The evidence gathered during the inspection determined a violation with an applicable standard or law. Standard #:	22VAC40-131-290-C
Description:	Violation: Based on a review of two children's records (FC-1 and FC-2) and an interview with an agency representative (AR-1), the agency failed to include required elements in the children's medical examinations. Evidence: 1) The medical evaluations for FC-1 dated 4/05/23 and for FC-2 dated 2/24/23 do not document whether the children are free from communicable disease, including tuberculosis. 2) In an email on 6//07/23, AR-1 acknowledged that the medical evaluations for FC-1 and FC-2 did not include evidence of freedom from communicable disease, including tuberculosis.
