Technical Assistance:
Communication occurred regarding filing required documents in case records. The agency utilizes an automated system to maintain official case records for children and foster homes and paper back up files. Documentation required by regulation should be placed in the official automated record.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary
due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced mandated renewal inspection was initiated on 8/18/2020 and concluded on 8/21/2020. The Director of Foster Care and Independent Living was contacted by email to initiate the inspection. The inspector emailed the Director of Foster Care and Independent Living a request for documentation required to complete the inspection. The agency reported 28 children in placement and 32 approved provider homes at the time of inspection. Three (3) foster child, four (4) foster home, and seven (7) personnel records were reviewed during this inspection. Background checks for all Board Officers were reviewed on 8/12/2020 during the renewal inspection of the agency?s Cedar Bluff office.

The Director of Foster Care and Independent Living participated in the entrance conference and remained available during the inspection. The Director of Foster Care and Independent Living, Regional Manager of Foster Care and Independent Living, two (2) Foster Care Supervisors, and two (2) Foster Care Specialists participated in the exit interview on 8/21/2020. One (1) violation of the Standards for Licensed Child Placing Agencies were cited.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review for Foster Child 3 (FC3), the agency failed to obtain and place in the record a birth certificate for the child.

Findings:

The record for FC3 did not contain a birth certificate. The findings were discussed during the exit interview. Staff 1, 2, and 4 (S1, S2, and S4) reviewed the record and did not locate a birth certificate. Staff located an email from their caseworker requesting the birth certificate from the caseworker with the local department of social services (LDSS) holding custody of the child. The LDSS caseworker responded to the email stating she would scan and email the birth certificate. No additional documentation was located to indicate the birth certificate was received. S1 acknowledged the findings.

Plan of Correction: Staff have secured a copy of the birth certificate from LDSS. Program Director will remind staff to make multiple requests of the LDSS regarding admissions documentation
