Comments:
A renewal inspection was initiated on 8/9/2021and concluded on 8/10/2021. The Regional Manager of Foster Care was contacted by telephone to initiate the inspection and reported that the current census was 24. The inspector emailed the Regional Manager of Foster Care a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster child, three (3) staff, and two (2) provider home records. An exit interview was conducted with the Regional Manager of Foster Care and two (2) Foster Care Supervisors on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description: Violation
Based on record review and interviews, the agency failed to ensure foster home 1 (FH1) maintained the key or lock to firearms and ammunition out of reach of children.

Findings:
1) The home study documented the firearms are stored in a locked box and ammunition is stored in a separate locked box out of reach of children in the foster parent?s bedroom.
2) The home study documented ?The key or combination to the locked locations for both firearms and ammunition is maintained in the foster father?s nightstand underneath some items hidden from view, out of reach of children.?
3) The findings were discussed during the exit interview. Staff 1, 2, and 3 (S1, S2, and S3) reviewed the home study, were interviewed, and acknowledged the information in the home study. S2 contacted the foster parents to determine if the nightstand or room in which the nightstand is located were kept locked. S2 reported the nightstand does not lock and the bedroom in which the nightstand is located is not kept locked.
4) S1, S2, S3 acknowledged the findings.
