Comments:
A monitoring inspection was conducted on 1/12/2022 from 2:40 p.m. to 5:00 p.m. and on 1/13/2022 from 9:00 a.m. to 2:20 p.m. The Foster Care Supervisor participated in the entrance conference and reported a current census of 29 children in placement and 30 approved provider homes. The inspector reviewed two (2) foster child, three (3) staff, and three (3) provider home records. An exit meeting was conducted on 1/13/2022 with the Director of Foster Care and Independent Living, Foster Care Specialist, and Foster Care Supervisor, where preliminary findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. An Acknowledgment of Inspection was completed.

The information gathered during the inspection determined non-compliance(s) with applicable standards or law. Violations were documented on the violation notice issued to the facility.
Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Based on record review, the agency failed to address required elements in the written assessment required by 22VAC40-131-250.O. for Foster Child 1 (FC1)

Findings:
1. A Placement Study/Social History for FC1 did not address information prior to acceptance regarding the child?s day care or nursery school or adjustment.
2. The child is not of school age.
3. The findings were discussed during the exit interview.
4. Staff 1, 2, and 3 reviewed the Placement Study/Social History document and did not locate the information.
5. Staff acknowledged the findings.

Description: Violation:

Based on record reviews, the agency failed to obtain tuberculosis screening reports containing required elements for the applicants in Foster Home 3 (FH3).

Findings:
1. The record for F3 contained tuberculosis screening forms for the applicants.
2. The medical professionals did not include a statement indicating the applicants were free of communicable tuberculosis in a communicable form on the forms.
3. The findings were discussed during the exit interview. Staff 1, 2, and 3 reviewed case record and tuberculosis screening documents.
4. Staff acknowledged the findings.
