Comments:
A renewal inspection was initiated on 8/23/2021 and concluded on 8/26/2021. The Regional Manager of Foster Care was contacted by telephone to initiate the inspection. The inspector emailed the Regional Manager of Foster Care a list of items required to complete the remote documentation review portion of the inspection. The Regional Manager of Foster Care reported a current census of 14 children in placement. The inspector reviewed two (2) foster child, three (3) staff, and two (2) provider home records. An exit interview was conducted with the Director of Foster Care and Independent Living, Regional Manager of Foster Care and Foster Care Supervisor on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description: Violation:

Based on record review for Foster Home 2 (FH2), the agency failed to consider all information prior to approving the foster home.

Findings:

1. Health Statements for both applicants were located in the record.
2. The medical provider completing the Health Statement for foster parent 2 (FP2) in foster home 2 (FH2) answered yes to the question ?In your opinion, does the individual have a physical, mental health, or substance abuse issues that will affect the care of foster or adoptive children?
3. The home study referenced the health statement for FP2 in FH2 and stated the medical provider ??did not note any concerns, issues, problems, or recommended limitation.?
4. The home study and other documentation in the record did not demonstrate consideration of the information.
5. References, case worker interviews with FP2 in FH2, and face to face visit with FP2 in FH2 were completed prior to approval and revealed no concerns with the FP2?s physical or mental health or potential substance abuse issues.
6. The findings were discussed during the exit interview. Staff indicated the foster parent self-reported having not physical, mental health, or substance abuse issues that would affect the care of foster or adoptive children
7. Staff acknowledged the findings.

Description: Violation:
Based on document review for personnel 3 (P3), the agency failed to obtain criminal history background check results within 3 years.

Findings:
1. Criminal history search results provided for P3 were dated 10/25/2017.
2. A criminal history search document dated 9/21/2020 stated the status of the search as ?transaction being processed.?
3. The findings were discussed during the exit interview.
4. Staff 2 (S2) contacted the agency?s Human Resources Specialist (S5) to determine if results were returned for the 9/21/2021 criminal search.
5. S5 emailed the licensing specialist and confirmed search results for P3 for the 9/212020 submission were not located. S5 submitted a new criminal history search for P3 on 8/27/2021.
