Comments:
A monitoring inspection was conducted on 1/11/2022 from 4:45 to 5:00 p.m. and on 1/12/2022 from 9:00 a.m. to 1:20 p.m. The Director of Foster Care and Independent Living participated in the entrance conference and reported a current census of 13 children in placement and nine (9) approved provider homes. The inspector reviewed two (2) foster child, three (3) staff, and two (2) provider home records. An exit meeting was conducted on 1/12/2022 with the Director of Foster Care and Independent Living and Foster Care Supervisor, where preliminary findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. An Acknowledgment of Inspection was completed.

The information gathered during the inspection determined non-compliance(s) with applicable standards or law. Violations were documented on the violation notice issued to the facility.
Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days.

Description: Violation:
Based on record review, the agency failed to obtain an annual physical examination report addressing required elements for Foster Child 2 (FC2).

Findings:
1) A written annual physical examination report for FC2 documented an exam date of 5/5/2021 and was signed by a medical professional.
2) The physical examination report did not address the child?s growth and development, visual acuity, auditory acuity, or allergies.
3) The findings were discussed during the exit interview and Staff 2 (S2) reviewed the exam report.
4) S2 acknowledged the findings.

Description: Violation:
Based on record review and interview, the agency failed to address required elements in the Discharge Summary for Foster Child 1 (FC1).

Findings:
1) The Discharge Summary for FC1 did not include the name of the person the child was discharged to.
2) The findings were discussed during the exit interview.
3) Staff 2 (S2) reviewed the Discharge Summary and acknowledged the name of the individual the child was discharged to was not documented. S2 provided the name of the individual to whom the child was discharged during interview.
4) S2 acknowledged the findings.
