Type of inspection: Renewal
An unannounced monitoring inspection was conducted on 08/30/2022 from 11:35 a.m. to 5:00 p.m. An entrance conference was held with the Foster Care (FC) Supervisor and Director of Foster Care and Independent Living. A preliminary findings review was held on 08/30/2022 with the FC Supervisor and Director of Foster Care and Independent Living.

The Acknowledgement of Inspection form was signed and left at the facility for the date of the inspection.

The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

An exit meeting was conducted with the Director of Foster Care and Independent Living on 09/06/2022.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-40-C
Description: Violation:
Based on review of documentation, the agency failed to establish that Board Officer 2 (B2) is of good character and reputation as defined in 22VAC80-10.

Findings:
1) References for B2 were not submitted with the renewal application.
2) The Licensing Specialist advised the Director of Foster Care and Independent Living that B2 is a new board officer and references were needed.
3) Findings were discussed during the preliminary findings review.
3) The Licensing Specialist communicated with the Human Resources Specialist to request references for B2.
4) The Human Resources Specialist informed the Licensing Specialist that references were not on file for B2.
5) Findings were discussed during the exit interview.

Standard #: 22VAC40-131-290-C-12
Description: Violation:
Based on record review, the agency failed to obtain the required immunization record for Foster child 1 (FC1).

Findings:
1) FC1 was placed in an agency home on 02/28/2022.
3) A medical examination report for FC1 did not include a copy of FC1?s immunization record.
4) The findings were discussed during the preliminary findings review.
5) S9 was interviewed and stated attempts were made to obtain the immunization records.
6) S9 reviewed the record for documentation of attempts to obtain the immunization record.
6) S9 provided a copy of an email sent to the custodial agency dated 06/10/2022.
7) Findings were further discussed at exit interview.

Standard #: 22VAC40-131-330-G-4
Description: Violation:
Based on record review, the agency failed to complete a private interview with Foster Child 1 (FC1) in March 2022.

Findings:
1) Case notes in the record for FC1 reflected contacts for the month of March including home visits, appointments and transportation (03/8/2022, 03/17/2022, 03/18/2022, 03/23/2022 and 3/30/22.)
2) The case notes did not demonstrate a private interview held with FC1 during March 2022.
3) The findings were discussed during the preliminary findings review and Staff 9 (S9) reviewed the case contacts.
4) S9 acknowledged the findings.
5) Findings were further discussed during the exit meeting.
