Type of inspection: Monitoring An unannounced monitoring inspection was conducted on 4/19/24. A site visit was made on the same date from 9:50 a.m. to 10:10 a.m. for the office inspection and entrance. The record review was completed virtually from 3:15 p.m. to 5:30 p.m. The initial finding was sent via e-mail. Number of children in care: 14 (6 in Independent Living) Number of approved provider homes: 9 Number of children?s records reviewed: 2 Number of provider homes reviewed: 1 An exit meeting was held on 4/22/2024. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
