Comments:
An unannounced monitoring inspection was conducted on January 9, 2020 from 9:05 a.m. to 2:30 p.m. The licensee reports 16 children in foster care and 21 approved provider homes. During this inspection two (2) foster child and five (5) foster home records were reviewed.

The Director of Foster Care and Independent Living and Administrative Assistant participated in the entrance conference and remained available during the inspection. An exit interview was conducted from 1:50 p.m. to 2:30 p.m. with the Director of Foster Care and Independent Living, a Regional Supervisor, and a Family Services Specialist. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the inspection.

There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interview, the agency failed to document the receipt of pre-service training in the record for Foster Home 3 (FH3).

Findings:

The Home Study for FH3 state ?Please see Pre-Service Orientation Log, signed by the parents, in the family?s file.? The orientation log in the record was blank. The record did not contain other documentation of the applicants' participation in training. The findings were discussed with agency staff during the exit interview. S2 stated she was present for each pre-service training session during the period and the applicants were present. S4 states she was also present and trained classes and confirms the applicants were present. Staff acknowledged the record did not contain the documentation.

Plan of Correction: Program Director will remind staff to ensure pre-service orientation logs are completed and present prior to approval of families
