Comments:
The agency reported seven (7) foster children in placement and seven (7) approved foster homes. During this inspection one (1) foster child, six (6) foster home, and three (3) personnel records were reviewed. An Administrative Assistant participated in the entrance conference and remained available during the inspection. A Regional Director remained available during the inspection and participated in the exit interview. The Foster Home Recruiter was interviewed during the exit interview. The exit interview was conducted from 3:10 p.m. to 3:45 p.m. An acknowledgment form was completed and left with the agency at the end of the inspection.

There was one citation for violations of the Standards for Child-Placing Agencies. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Based on record review and interview, the agency failed to document consideration of all required elements prior to recommending the type of home best suited to Foster Child 1 (FC1).

Findings:

The case record for FC1 contained a placement study document the agency uses to document information regarding placement type best suited to a child. The area to address this information was blank. The findings were discussed with S1 during the exit interview. S1 participated in the child?s intake and indicated required information was considered. S1 reviewed the document and looked through the case record to determine if additional documentation was available. Documentation was not located and S1 acknowledged the findings.

Plan of Correction: Program Director will remind staff of the importance of timely and thorough documentation of placement information.

Staff will ensure documentation for this case is up to date.

S1 participated in the child's intake and indicated matching factors were considered in selecting the home for the child. . S1 reviewed the document and looked through the case record to determine if additional documentation was available. Documentation was not located and S1 acknowledged the findings.
