Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An announced monitoring inspection was initiated on 5/27/2021 and concluded on 5/28/2021. The agency reported six (6) children in foster care and 12 approved provider homes. During this inspection one (1) foster child and two (2) provider home records were reviewed.

The Director of Foster Care and Independent Living responded to the notice of inspection, provided access to records, and remained available during the inspection. The Director of Foster Care and Independent Living, Regional Manager, Foster Care Supervisor, and Foster Care Caseworker participated in the exit interview on 5/28/2021.

There was one (1) citation for a violation of the Standards for Licensed Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review for foster child 1 (FC1), the agency failed to document all required information in the child?s Social History.

Findings:
1) The Social History for FC1 documented the occupation and education of FC1?s grandparents, aunts, and siblings as unknown.
2) The Social History for FC1 documented the location of one uncle and did not state his education or occupation.
3) The Social History for FC1 documented ?Employment Status and History of child?s siblings: The employment status and history of child?s sibling is unknown. This information will be requested from DSS? and ?Educational and Occupational History of child?s siblings: The educational and occupational history of the child?s sibling is unknown. This information will be requested from DSS.?
4) The findings were discussed during the exit interview. S3 advised staff had requested the information from the child?s grandmother but had not received a response.
5) Staff acknowledge the information was not documented in the Social History.

Plan of Correction: social history addendum will be completed
to reflect previous attempts at communication
with grandmother and remaining sections will be
as additional information is obtained. 6/4/2021
Staff will be reminded to document in placement
study to obtain information from birth family and
prior custodians and to document unsuccessful
attempts in the placement study.
