Organization and Administration Provider Homes Children's Services Background Checks

Comments:

An unannounced onsite monitoring inspection was conducted on 3/8/2022 from 11:00 a.m. to 2:00 p.m. The Lead Foster Care Caseworker participated in the entrance conference and reported a current census of eight (8) children in placement and nine (9) approved provider homes. No new staff were hired since the last inspection. The inspector reviewed one (1) foster child and one (1) provider home records. Office setting, capacity, and caseloads were inspected. An exit meeting was conducted on with the Regional Manager, Foster Care Supervisor, Lead Foster Care Specialist, and Foster Care Specialist on the date of inspection where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law. Violations are documented on the violation notice issued to the facility. Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on record review for Foster Child 1 (FC1), the agency failed to place in the file of FC1 a written assessment that contains all required elements specified in 22VAC40-131-250 G within 30 days of the child?s placement in a foster home. Findings: 1) The record for FC1 contained a Placement Study/Assessment/Social History document that addressed required elements in 22VAC40-131-250.G that was signed 65 days following the date the child was placed in a foster home. 2) The findings were discussed during the exit interview. 3) Staff 3 (S3) acknowledged the findings.
