Comments:

An unannounced, mandated, monitoring inspection was completed on September 6, 2019 from 12:00 am to 2:00 pm at Siena Adoption Services located at 4141 N. Henderson Road, Arlington, Virginia 22203. Currently, there three approved provider homes waiting for adoptive placement. During this inspection, two adoptive home records were reviewed. No personnel records were reviewed as no new staff were hired since the most recent inspection. There was one citation for a violation of the Standards for Child-Placing Agencies. It was a key, health and safety standard. The Case Worker and the Case Work trainee were available for the inspection and present for the exit interview at the end of the inspection. An Acknowledgement Form was signed by the Case Worker on this date. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description:	Violation: Based on a review of the record an an interview with agency representatives, the agency failed to obtain a tuberculosis screening on a household member as required. Evidence: 1) AH-2 (adoptive home 2) was approved on 3/26/19. The adoptive parents received tuberculosis screenings/tests but their biological child did not. 2) The agency representatives acknowledged that this was an oversight.
Plan of Correction:	The case worker will ensure that all household members that come into contact with children receive a TB screening/test.
