Comments:

A monitoring inspection was initiated on 06/18/2024 and concluded on 06/21/024. The Caseworker was contacted initiate the inspection. The Caseworker reported that no children were in placement at the time of inspection. A list of documents required to complete the inspection was provided to the Director. Five provider home records were reviewed. A review of findings was conducted 6/20/2024 and exit on 6/21/2024 where an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Inspection findings will be posted to the Department?s public website within 5 days of receipt of the inspection documents. The department's inspection findings are subject to public disclosure. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Description:	Violation: Based on a review provider home record documentation and interview, the agency failed include all required elements on the Certificate of Approval (COA) for Provider Homes 1 and 2 (PH1 and PH2). Evidence: 1) A COA for PH1 was signed and dated 10/26/2023. The titles of the individuals signing the COA were not included. 2) A COA for PH2 was signed and dated 6/16/2023. The titles of the individuals signing the COA were not included. 3) Staff 1 (S1) was interviewed, and findings were discussed. 4) S1 acknowledged the findings.

Description:	Violation: Based on a review of provider home record documentation and interview, the agency failed to obtain a required background check within 30 days of Household Member 1 (HH1) in Provider Home 1 (PH1) turning 14 years of age. Evidence: 1) HH1 turned 14 on 9/24/2023. 2) PH1 was approved 10/26/2023. 3) The central registry search results for HH1 were not located in PH1? record. 4) Staff 1 (S1) was interviewed and indicated she would need to discuss with the staff member who completed the home study. 5) S1 emailed the licensing inspector and stated ??it was not completed??
