Discussed within 30 days of the date of admission as it pertains to the development of the individual service plan as noted in 22VAC40-131-440.H.

Comments:
A monitoring inspection was initiated on 9/23/21 and concluded on 9/30/21. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was nine (9) youth. The Licensing Specialist emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection.

The following is a listing of the activities for this inspection:
Reviewed one current resident record. Four personnel records were reviewed, which consist of the new staff members hired since the May 2021 inspection. The Manager of Compliance and Development was interviewed on 9/28/21.

Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Violations:
Standard #: 22VAC40-131-140-F-2
Description: Violation: Based on interviews with staff and review of staff?s, S1?s, personnel record, the agency failed to hire a person to serve as a case worker who possesses a baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families.
Findings:
1) S1 has occupied a caseworker position as a Life Skills Coach for 55 days.

2) S1?s resume and the Degree Verify Certificate obtained by the agency documents a baccalaureate degree in
Human Development and Family Studies.

3) S1 does not possess one year of experience in providing casework services to children and families.
a. S1?s resume shows seven months of experience in providing casework services to children and families.

4) The Licensing Specialist interviewed S5 about S1?s qualifications.
a. The Licensing Specialist advised S1 does not possess one year of experience in providing casework services
to children and families.
b. The Licensing Specialist reviewed the definition of ?casework? in the ?Standards for Licensed Child-Placing
Agencies? with S5.
c. S5 reviewed S1?s resume and agreed with the Licensing Specialist?s findings.
