Comments:

This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 11/6/20 and concluded on 11/23/20. The Executive Director was contacted by telephone on 11/6/20 to initiate the inspection. The Executive Director reported that the current census was three (3) youth. The Licensing Specialist emailed the Executive Director a list of items required to complete the inspection. The following is a listing of the activities for this inspection: Reviewed one current resident record. Four personnel records were reviewed, which consist of new staff members hired since the December 2019 inspection. The Executive Director was interviewed on 11/18/20. The Human Resources Compliance and Training Consultant was interviewed on 11/18/20. Additional information was requested from the Human Resources Compliance and Training Consultant on 11/19/20. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.
