Inspection of the office setting and conditions 10/20/23 from 11:15 AM ? 5:26 PM ? Inspection conducted along with the YouthQuest-Virginia Beach (File #: 369) ? Renewal Inspection 10/24/23 from 11:01 AM ? 4:18 PM ? Review of personnel files along with other YouthQuest locations The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: 7 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of youth records reviewed: 1 Number of interviews conducted with staff: 3 Additional Comments/Discussion: An inspection of the office setting and conditions for this agency was conducted at this location. An entrance conference was held at YouthQuest?s other Virginia Beach location (File #: 369) on 10/20/23 with the Regional Supervisor. Also, a youth?s electronic record was reviewed on this date. The Regional Supervisor was available and accessible during the inspection. A preliminary findings meeting was held with the Regional Supervisor and the Executive Director (by phone) before leaving the agency. Staff records were reviewed at the corporate office in Richmond on 10/24/23. The HR Business Partner was available and accessible during this inspection. A preliminary findings meeting was held with the HR Business Partner. An exit meeting was conducted on 10/31/23 to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
