Comments:

An unannounced monitoring inspection was completed by the Licensing Specialist on 1/29/20 from approximately 10:59 a.m. to 4:26 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this facility. Also, an unannounced monitoring inspection was conducted by the Licensing Specialist on 1/30/20 from approximately 11:10 a.m. to 1:46 p.m. at the facility. The current census is four (4) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Two staff records of the newest staff members hired since the July 2019 monitoring inspection were reviewed. On 1/29/20, the Human Resources Compliance and Training Analyst was available and accessible during the inspection. An exit meeting was conducted with the Human Resources Compliance and Training Analyst and the Executive Director at approximately 3:41 p.m. The Acknowledgment of Inspection form was signed and left at the corporate office. On 1/30/20, the Regional Supervisor and the Senior Life Skills Coach were available and accessible during the inspection. An exit meeting was conducted with the Regional Supervisor at approximately 1:37 p.m. The Acknowledgement of Inspection form was signed and left at the facility. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.
