Comments:

An unannounced monitoring inspection was conducted onsite on 9/17/2019 from 10:30 to 12:25 p.m. Personnel records were reviewed remotely on the same day from 1:45 p.m. to 2:15 p.m. The agency reports five youth placed in Independent Living. Two new staff members were hired since the last inspection. During this inspection one youth and two personnel records were reviewed. An entrance conference was conducted with the Program Development Specialist and a Life Skills Coach Trainee. The Program Development Specialist and the Program Director were available during the inspection. The Program Director participated in the exit conference which was conducted on 9/17/2019 from 12:05 p.m. to 12:25 p.m. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the inspection. There were no citations for violations of the Standards for Child-Placing Agencies.
