Comments:

An unannounced renewal inspection was completed by the Licensing Specialist on 3/17/22 from 10:12 a.m. to 3:12 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this agency. Also, an unannounced monitoring inspection was conducted by the Licensing Specialist on 3/23/22 from 2:09 p.m. to 4:50 p.m. at the agency. The current census is one (1) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. One staff record was reviewed. On 3/17/22, the Manager of Compliance and Development was available and accessible during the inspection. This staff member was interviewed on the same date in order to discuss the staff record. The Acknowledgment of Inspection form was signed and left at the agency. On 3/23/22, the Assistant Director was available and accessible during the inspection. This staff member was interviewed on the same date in order to provide clarification about a particular document. The Acknowledgment of Inspection form was signed and left at the agency. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website.
