Comments:
Type of inspection: Monitoring
Licensing inspector was on-site at the facility May 8, 2024, 11:30 AM to 2:30 PM. A preliminary findings review was held on the date of the inspection. The Acknowledgement of Inspection form was signed and left at the facility on May 8, 2024.
An exit findings review was completed on May 15, 2024
Number of youth in care: 6 (ILA)
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Number of staff records reviewed: 2
Number of interviews conducted with staff: 2

Additional Comments/Discussion:
An entrance and preliminary conference were held at the agency on 05/08/2024. The Program Manager was onsite and available throughout the inspection. Additionally, the IL Director was available via virtual contact. The HR Business Partner and IL Assistant Director were accessible during the inspection and granted access to staff records and electronic health records.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Based on review of Y1?s record, the agency failed to clearly show Y1?s involvement in developing the individualized service plan.

Findings:
1) In review of Y1?s record on 05/08/24 it was observed in the record that the Transitional Living Service Plan (TLSP) dated 10/10/2023 was filed; however, there was no evidence observed in the record that demonstrated Y1?s involvement in developing the service plan.
2) The signature page of the TLSP reflects the signatures of the parties who were involved in the development of the TLSP; however, the line item where Y1?s name/signature would be included, was not reflected on the document.
3) The day of the inspection (05-08-24) S1 & S3 acknowledged the findings indicated above.

Findings:

3) The day of the inspection (05-08-24), the Licensing Inspector inquired as to whether there was a reason as to why Y1 did not sign the document and in acknowledging Y1?s involvement, development and receipt of a copy of the TLSP. After allowing time for the Provider to research during the inspection, S1 returned and indicated that there was no reason found or discovered as to why Y1?s signature was not a fixed to the TLSP to demonstrate Y1?s involvement.

4) The findings were discussed during the preliminary review. S1 and S3 acknowledged the findings.
