Discussed face-to-face visits in the youth?s place of residence within a calendar year as noted in standard 440.M.3.

Comments:
An unannounced renewal inspection was conducted by the Licensing Specialist on 11/20/19 from 9:58 a.m. to 4:36 p.m. at the corporate office in Richmond, where the staff records are maintained, for the purpose of reviewing staff records for this facility. Also, an unannounced renewal inspection at the facility was completed by the Licensing Specialist on 11/22/19 from 12:32 p.m. to 3:18 p.m. The current census is seven (7) youth.

The following is a listing of the activities for this inspection:
One youth record was reviewed. Five staff records were reviewed, which consist of the new staff members hired since the May 2019 inspection. Background checks for the Board Officers were previously reviewed during this fiscal year.

On 11/20/19, the Human Resources Compliance and Training Analyst was available and accessible during the inspection. An exit meeting was conducted with the Executive Director (accessible by video conferencing), the Human Resources Compliance and Training Analyst, and the Director of Human Resources and Talent Management at approximately 4:36 p.m. The Acknowledgement of Inspection form was signed and left at the corporate office.

On 11/22/19, the Assistant Director was available and accessible during the inspection. An exit meeting was conducted with the Assistant Director and the Program Manager at approximately 2:43 p.m. The Acknowledgement of Inspection form was signed and left at the facility.

Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Violations:
Standard #: 22VAC40-131-140-F-2
Description: Violation: Based on interviews with staff and review of staff?s, S4?s, personnel record, the facility failed to hire a person to serve as a case worker who possesses a baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families.
Findings:
1) S4?s employment application and resume documents a baccalaureate degree in social work.

2) S4 does not possess one year of experience in providing casework services to children and families.
a. S4?s resume shows six months of experience in providing casework services to children and families.

3) The Licensing Specialist interviewed S7 and S9 about S4?s qualifications.
a. The Licensing Specialist advised S4 only has six months of experience in providing casework services to children and families.
b. S7 and S9 reviewed S4?s resume. They agreed with the Licensing Specialist?s findings.
c. S7 and S9 stated S4 should have been hired as a case worker trainee.

Plan of Correction: Executive Director will ensure the Human Resource department as well as YouthQuest management team is fully aware of the qualifications of a Life Skills Coach (case worker). Addendum life skills coach trainee job description has been placed in file effective 12/5/19.
