Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/16/22 from 2:00 PM ? 4:06 PM and 6/28/22 from 12:40 PM ? 4:27 PM. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of children/youth in care: 4 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 1 Number of staff records reviewed: 2 Number of interviews conducted with staff: 3 Additional Comments/Discussion: An entrance conference was held at the agency on 6/16/22. Also, on the same date, a youth?s record was reviewed at the agency. The Regional Supervisor was available and accessible during the inspection. The Assistant Director and the Executive Director (by video call) were interviewed on the same date in order to provide clarification about several documents reviewed. Staff records were reviewed at the corporate office in Richmond on 6/28/22. The Manager of Compliance and Development was available and accessible during the inspection. This staff member was interviewed on the same date in order to provide clarification about several documents reviewed. An exit meeting was conducted on 6/30/22 to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
